Our Social Prescribers are link workers that have access to a wide range of local agencies, best suited to help with your overall health and wellbeing.
What could you see a Social Prescriber for?
– Helping to access services from social care, pharmacies, allied health professionals, fire service, housing associations, job centres, employment and/or volunteering support.
– Needing help with debt, benefits and housing.
– Loneliness, accessing social groups, mental health and support groups.
If you would like to submit a self-referral, head over to your practice website and complete the online form.
You can also contact your practice reception team to ask for an appointment, stating the reason why you would like to be seen and the best telephone number to call you on and one of the Social Prescribers will arrange an appointment for you.
