Combined Healthcare’s online All-Age Wellbeing Portal self-referral system for adults and people with learning disabilities

We are pleased to announce that the North Staffordshire Combined Healthcare NHS Trust’s online All-Age Wellbeing Portal self-referral system for adults and people with learning disabilities is now live. This is a new capability within the Portal, enabling patients with the opportunity to refer themselves using the new online referral form. It also allows carers and professionals to refer on behalf of the patient, where consent has been provided.

 

The online self-referral system is an innovative enhancement to the All-Age Wellbeing Portal which has helped to empower and enable the self-management of mental wellbeing for its users, whilst providing relevant information, guidance and support from the Trust and other partner organisations.

 

Self-referral allows responsive access into Combined Healthcare’s services utilising digital technologies that work alongside the latest research-based assessments, ensuring the right level of help, by the correct service based on the presenting complexities.

 

The online referral forms can be located within the Wellbeing Portal via:- https://combinedwellbeing.org.uk and then accessing either the Adult Wellbeing or Learning Disabilities tabs which provide a drop-down box with the online referral option.
